New Patients

Your first visit to Norridge Foot & Ankle Clinic involves a few special steps so that we can get to know you. 

Please know the name of your primary care physician and the date that you last saw them. If you are diabetic, please know your last Hemoglobin A1C value. In addition, please bring the following items when coming to your first appointment:

• Identification
• Insurance Card(s)
• List of current medications
• List of medical conditions, surgical procedures, and allergies
